We are excited to meet you and your child!!!

Here are the steps to get started:

1. Call (208)255-7337 to discuss your child’s needs with our front desk staff and schedule a Welcome Visit.
2. Attend your Welcome Visit.  At your welcome visit, we will have you fill out all necessary paperwork, review your insurance benefits, review our attendance policy, and give you a tour of our facility.  Your child is not required to attend the Welcome Visit but is certainly welcome to attend.  We will schedule a time for your child’s evaluation at the Welcome Visit if appointment times are available; if no openings are currently available, we will place your child on a waiting list and call you as soon as we can get them in for an evaluation.
3. Please bring the following to your Welcome Visit:  insurance card, ID, and any information regarding your child’s condition that may help you fill out the paperwork.
4. Referrals:
   • PT/OT:  a referral is required for all Medicaid patients and some private insurances.  Please check with your insurance company to verify whether a referral is required.
5. First appointment:  Please have your child wear comfortable clothing they can move easily in.  Bring any equipment or braces that they use regularly.  We can’t wait to meet you!!!
